1. Service Description

DenyBack ("Service," "we," "us," or "our") is a document preparation service that generates health insurance appeal letters, follow-up letters, state insurance commissioner complaints, and external review requests based on information you provide. We are not a law firm, and our Service does not constitute legal advice, legal representation, or an attorney-client relationship. The documents we generate are templates informed by publicly available insurance regulations and the details you supply.

2. User Responsibilities

You are responsible for the accuracy and completeness of all information you provide through our intake form. You are responsible for reviewing all generated documents before submitting them to your insurance company, state regulatory agencies, or any other party. You acknowledge that you are the sole decision-maker regarding whether and how to use the documents generated by our Service. You agree not to use our Service for any unlawful purpose or in any manner that could damage, disable, overburden, or impair the Service.

3. Payment Terms

The Service costs $29 per case as a one-time payment. Payment is processed securely through Stripe. Once your documents have been generated, the payment is non-refundable. If document generation fails due to a technical error on our part, you will receive a full refund. We do not offer subscriptions, recurring charges, or hidden fees.

4. Limitation of Liability

To the maximum extent permitted by applicable law, DenyBack and its officers, directors, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, use, or goodwill, arising out of or in connection with your use of the Service. Our total liability for any claim arising from or related to the Service shall not exceed the amount you paid for the specific case in question.

5. No Guarantee of Outcomes

DenyBack does not guarantee that any appeal, complaint, or review request generated through our Service will result in the reversal of an insurance claim denial. Outcomes depend on numerous factors beyond our control, including the specific facts of your case, your insurance plan terms, applicable state and federal regulations, and the decisions of insurance company reviewers, independent review organizations, and state regulators.

6. Data Handling

We collect and process personal and health-related information solely for the purpose of generating your appeal documents. Please refer to our Privacy Policy for detailed information about how we collect, use, store, and protect your data.

7. Modifications to Terms

We reserve the right to modify these Terms of Service at any time. Changes will be effective when posted on this page with an updated "Last updated" date. Your continued use of the Service after any changes constitutes acceptance of the revised terms. We encourage you to review these terms periodically.
